FOOTBALL SPORTS REGISTRATION
Complete the online registration form ONLY ONCE PER SCHOOL YEAR & during the season of your 1st sport played!
ALL football athletes MUST receive a FOOTBALL Sports Permit to be eligible to participate on the first official date of practice on Thursday, September 24! Please choose the criteria below that fits your situation, and complete and submit the required items. Deadline to register is Wednesday, September 23! NO REFUNDS will be given if you quit once you begin practicing!
1. Signed "parent/athlete signature form" (from the online registration confirmation emailed to you)
2. Signed "CHSAA Competitor Brochure / Covid-19 Risk Signature Form (read, print and sign page 3 from
below printable forms)
3. Updated physical
4. Payment of Sports Fee: $150 ($200 for football, no fee for team managers, cheer and poms) for in-
district students. $225 ($300 for football) for out-of-district students. – NO REFUNDS, with the
exception of sports cancellation due to Covid-19!
5. If you are a freshman athlete or did not participate in sports at WHS last school year, you must create a
ONE-Time CHSAA "ATHLETE" (not parent) account by signing up on the CHSAA Eligibility platform at
planeths.com.
IF you are an INTERNATIONAL STUDENT, or a TRANSFER STUDENT that attended another high school other than Windsor High School for a PORTION of the 19-20 school year, please complete the online registration form (link below) and then complete and submit the following to Sandee Legino, Athletics Secretary:

5. If you are a freshman athlete or did not participate in sports at WHS last school year, you must create a
ONE-TIME CHSAA "ATHLETE" (not parent) account by signing up on the CHSAA Eligibility Platform at
planeths.com. Then the parent will need to create a parent account and link it to the student
account.
6. Sign in to your newly created ATHLETE planeths.com account, click on the Transfer Forms tab
complete the appropriate CHSAA Transfer Eligibility Form 9, 9b or 9c, and process electronically.
Please follow the instructions carefully. You will be required to obtain electronic signatures from your
"sending school's" Athletic Director and to provide all corresponding required documents (i.e. copy of
proof of sale of home or rental/lease agreements, copy of proof of purchase of home or rental/lease
agreement, etc.). Please call Sandee Legino at 686-8104 for an appointment or for assistance in
completing the transfer process.

6. Completed Non-WHS Student Athlete General Information Form (printable form below) - must be
completed in its entirety and required signatures obtained.

and required signature obtained.
